Hafal Crossroads Day Centre

The Machynlleth Day Centre supports the independence and well-being of older people by providing a user led service that aims to reduce social isolation and loneliness of individuals within their local community. The Centre offers individuals an opportunity for companionship and friendship in a stimulating, safe and therapeutic environment.

Hafal Crossroads Domiciliary Home Care Service

In Powys the Hafal Crossroads Domiciliary Home Care service provides practical support to promote individual’s wellbeing and independence enabling them to remain living at home in their local community.
This service includes working in partnership with local community nursing teams to provide person-centred end of life domiciliary care and emotional support for individuals who wish to remain being cared for in their own homes. End of life care support is commissioned by Powys Teaching Health Board (PTHB).

All support and advice is provided based on promoting each individuals’ wellbeing, ensuring the needs, desires and personal preferences of the individual form the basis of all support provided.

Private funded or via spot contract following an identified assessment of need completed by the local authority.

Powys Urgent Response Service at Home (P.U.R.S.H)

Hafal Crossroads work in partnership with PTHB to provide short term (up to 5 days) domiciliary support and health care within peoples’ own homes, in emergency situations when hospital admission is deemed inappropriate, reducing the number of avoidable admissions of Powys residents. There is no charge to individuals for this service.

This service is available to individuals (18 years of age and above), living in Powys (or close to the county border), registered with a Powys based GP.

Referrals are via health professionals such a GP/Shrop Doc and community nursing teams only. Self-referrals are not accepted.

Powys Carers Respite and Advice Service            

The Powys-based Hafal Crossroads Carers Respite and Advice Service provides practical support (respite) and advice to unpaid carers of all ages, enabling them to sustain their caring role and take a meaningful break from their caring responsibilities.

All support and advice is provided based on promoting each individuals’ wellbeing, ensuring the needs, desires and personal preferences of both the carer and the individual they cared for, form the basis of all support provided.

This is available via private funding, for free via time limited PTHB funding to prevent carer breakdown and support health of carers e.g. attend health appointments. Also via commissioned services by the Local Authority following a Carer Assessment.

Self-referrals welcome as well as from family/friends and other third sector organisations and professionals.

Support provided can include:

  • Practical support (respite)
  • Advice, guidance and signing posting to other sources of support
  • Promoting opportunities for social inclusion
